Bridging the Gap provides services to young adults between 16-24 years who are experiencing mental health concerns, issues with well-being, and barriers to service. We work together with the individual and their supports to help them access services, learn new skills and strengthen their support network.
Individuals served by the program often report feeling alone and struggle in their relationships with others. Many have difficulty accessing the services they require (e.g., affordable housing, mental health services), due to utilization problems and systems barriers. Individuals may struggle in ways that make it difficult for them to function well in a number of settings, and in ways that compromise their safety.
Services are customized and flexibly delivered based on the needs of the individual. These may include education and skill-based teaching, assistance with accessing and participating in community and professional services, and emotional and practical support. The program delivers individual and group skills training using the evidence-based modality Dialectical Behaviour Therapy (DBT), which focuses on crisis management, emotion regulation, interpersonal effectiveness and how to live in the moment. In addition, the program works closely with a number of community partners, to reduce service barriers and enhance service coordination.
By meeting the individual where they are at and customizing each individual’s service plan, we are able to motivate and help move persons served towards success.
Bridging the Gap believes that individuals have the ability to experience wellness and independence, and that they have the right to attain a satisfying quality of life.
