Patch: Community and Family Parenting/Caregiver Level III Facilitator

The Patch program has recently restructured and we are adding a number of positions, including a full-time Community and Family Parenting/Caregiver Facilitator.

Patch is best described as a neighbourhood hub that provides a wide range of integrated services to children, families and their community. The central goals of the Patch hubs are to decrease poverty, increase health and wellness of children and families, and build thriving communities.

The Patch hub acts as a central access point for services that reflect a holistic response to the effects of poverty and that are provided in a way that is reflective of the local needs of the community. 

The Community and Family Facilitator (CFF) is part of a team responsible for the day to day operation and walk-in appointments at the Patch hub, and is assigned to deliver one of four specialized services: Parenting/Caregiver, Connections and Gatherings, Employment and Education, or Financial Empowerment. This role is based on a 40 hour workweek and requires a willingness to work in a non-structured work schedule, including some evenings and weekends, to fit family and/or individual and program needs. 

The Community and Family Parenting/Caregiver Facilitator works with families interested in receiving parenting support/strategies, such as education regarding developmental ages and stages, and system navigation (i.e., connecting with schools, FSCD, etc.). The Parenting/Caregiver Facilitator also facilitates Social Emotional Learning groups for designated children. Staff are expected to meet in the family’s home or in other community locations such as court or Alberta Works to offer support during appointments.

The Community and Family Level III Facilitator is a leadership level position that also provides front line service and support. The Level III will work as part of the Patch leadership team in providing supervision of staff, program level administrative responsibilities, oversight of the Patch Hub day to day operation and implementation of the program’s services. 

The position has a high degree of independence and autonomy. Staff will arrange and oversee their schedule in order to meet the needs of their families, the program and their own well-being. 

Duties and Responsibilities

  • Oversee the day to day operation and program delivery at the designated Patch Hub (e.g., ensure coverage is adequate for the hub schedule).
  • Provide mentoring/training and support to staff in order to deliver endorsed interventions and services to families and/or individuals (persons served).
  • Actively participate in program senior level meetings/initiatives.
  • Lead the required/identified specialization accompanying this role.
  • Responsible for health and safety standard practices, providing kids snacks, completing necessary referrals, sharing resource information with persons served, assisting persons served with understanding forms/letters (English Language Learners) and registering for subsidies,  assisting/teaching persons served faxing, and making phone calls.
  • Provide support services to families including skill building, crisis management, advocacy and community resourcing in a variety of settings (i.e., hub, school, in-home and community).
  • Provide support for benefits, system navigation, and access to subsidies.
  • Supporting financial empowerment strategies, including being trained as a financial coach.
  • Manage and carry a caseload (be a Service Lead) for a designated number of families/individuals.
  • Coordinate and co-facilitate skill based training at the individual and group level (i.e., parent, child and/or youth groups).
  • Assist in ensuring that the basic needs of persons served are being met (i.e., food, housing, transportation, employment, daycare, social support networks).
  • If applicable and available in the particular communities, work collaboratively with surrounding schools to support youth education and health.
  • Keep thorough and up-to-date records of person served contact and the services provided.
  • Travel to and from various community resource providers and services to pick up program supplies.
  • Be prepared to work at any of the Patch hub/locations in order to offer support/coverage as required and directed by supervisors.
  • Be involved in or lead program, agency or community committees.
  • Perform outreach duties: go door to door to engage and identify needs of persons served; deliver newsletters to persons served. 
  • Maintain hub cleanliness.
  • Perform other tasks and duties as required.

Skills and Qualifications

  • Qualified at level III (5 years experience in a supervisory and/or leadership role preferred)
  • Completed leadership training course (e.g., Donna Arvidson at Hull Services)
  • Has supervision experience (including promoting and encouraging staff with professional development and able to address challenging situations using a solution focused approach).
  • Able to promote and deliver the program’s framework, adhere to and encourage program and Agency standards and with support from the Coordinator oversee the operation and management of the Patch Hub. 
  • Confident, reliable, self aware, receptive to feedback (and able to deliver it), and strong communication skills.  
  • Liaising with community partners.
  • A minimum of Bachelor's Degree or Diploma in the Social Sciences or related field is required. 
  • Unless otherwise stipulated, willingness to attain CYCAA certification within a 4 year period.
  • Relevant experience in working with children, youths and their families, and adult individuals. 
  • Knowledge of crisis intervention and family education. 
  • Strong communication skills, public speaking, chairing meeting and professional writing (e.g., case notes, goals, and presentations).
  • Able to effectively manage working in a non-traditional workspace and contribute to an inclusive and diverse healthy workplace environment. 
  • Demonstrated knowledge of community engagement, resources and services. 
  • High degree of self-awareness and a strong ability to model appropriate behaviours and exercise good judgment in working with persons served and co-workers. 
  • Clear Police Check including Vulnerable Sector Check; Child Intervention Record Check
  • Must have valid driver's license and reliable transportation and must be willing to drive agency vehicles. 
  • Be able to lift and carry 10-25kg (food bank, I Can for Kids, etc.)
  • Hold adequate automobile liability insurance to transport persons served in personal vehicle, when required.
  • Intermediate knowledge of Microsoft Office, computer and typing skills.
  • Completion of ASIST, First Aid, Financial Coaching, CRA tax return training and Certified with CYCAA and/or holds an RSW, would be considered an asset.
